Answer:  You note macula threatening (“mac-on”) retinal detachment which is confirmed by B-scan.  You notify the on call retina fellow. On dilated exam you also note pigmented cells in the anterior vitreous off the right eye.

What is the significance of these pigmented cells?
Insignificant 
This is called Shafer’s sign. It is present when there is a retinal tear as pigmented cells from the torn RPE can migrate into the vitreous.
This is an indicator of an inflammatory cause of the retinal detachment

Pars plana vitrectomy
Scleral buckle
Pneumatic retinopexy
