History of Present Illness (HPI)

A 54 yo female with hx of malignant HTN and ESRD, s/p renal transplant 1 year ago, presents with decreased vision in both eyes over the past few months. Prior to this she reports having excellent vision in both eyes and only needed glasses for reading. She now feels she is “blind” and cannot ambulate without assistance from someone. She denies eye pain, redness, discharge, flashes, and floaters. Her HTN is now well controlled and renal allograft is functioning well. Of note, she was on oral prednisone for a few months following her renal transplant, but has now been able to taper off.
